Eligibility

1. Must have one (or more) of the following health problems:

Recent Fall
Past History of Falls
Risk of Falls
Fear of Falls

2. Must live in one of the following areas:

Mildura
Irymple
Merebein
Red Cliffs

3. Must have a referral from one of the following:

Hospital or community-based health professional
General Practitioner (GP)
Self-referral or referral from family members are accepted
