• Falls and Balance Clinic (1:1 appointment)
• Health Independence Program - Community Rehabilitation Service 
• Face-to-face

Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall*
• Past history of falls
• Risk of falls
• Fear of falls

*If cardiac or neurological origin, refer directly to a cardiologist or neurologist as appropriate

2. Must live in one of the following areas:

• City of Darebin
• City of Banyule
• City of Nillumbik
• Referrals outside of service catchment areas are possible but referral to nearest/local services are encouraged

3. Must have a referral from one of the following:

• Hospital or community-based health professional
• General Practitioner (GP)
• Self-referral or referral from family members are accepted

Cost

There is no cost to attend the Falls and Balance Clinic.
