• Care Co-ordinator

Eligibility

1. Must have one (or more) of the following health problems:

Falls Clinic:

• Recent Fall
• Past History of Falls
• Risk of Falls
• Fear of Falls

Falls Program:

• As above

2. must live in one of the following areas:

Falls Clinic:

• Albury
• Wodonga
• Lockhart
• Indigo
• Federation
• Towong
• Greater Hume

Falls Program:

• As above

3. must have a referral from one of the following:

• Hospital or Community-based health professionals 
• General Practitioner (GP)
• Self-referral and/or referral from family members are accepted

Cost

There is no cost to attend the Falls Clinic or Falls Prevention Program.
