Must have one (or more) of the following health problems: Recent fall Past history of falls Risk of falls Fear of falls 2. Must live in one of the following areas: Bass Coast Shire 3. Must have a referral from one of the following: Hospital or community-based health professional General Practitioner (GP) Self-referral or referral from family members are accepted Cost The cost to attend the Falls and Balance Clinic is based on individual circumstances.
