Accredited and Registered Fitness Instructor (Certificate 3 & 4 and a Diploma in Fitness instruction)

ELIGIBILITY

1. Must have one (or more) of the following:

Recent fall
Past history of falls
Risk of falls
Fear of falls
65 and older age criteria

2. Must live in one of the following areas:

City of Boroondara

3. Must have a referral from one of the following:

Hospital-based or Community-based health professional
Consultant
Registrar/Resident
Nurse
Allied Health Professional
Care Co-ordinator
Aged Care Assessment Service
Case Manager
General Practitioner (GP)
Self-referral or referral from family members are accepted
