Must have one (or more) of the following health problems: Recent fall History of falls Risk of falls Fear of falls 2. Must live in one of the following areas: City of Boroondara City of Yarra City of Darebin (Alphington, Fairfield, Northcote, Thornbury, Westgarth) 3. Must have a referral from one of the following: Hospital or community-based health professional General Practitioner (GP) Self-referral or referral from family members are accepted Cost There is no cost to attend the Falls and Balance Assessment Clinic.
