• Falls Prevention Program (2-hour sessions per week for 10-weeks)

Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls

2. Must live in one of the following areas:

• Foster and neighbouring towns (such as Stony Creek, Fish Creek, Walkville, Welshpool)

3. Must have a referral from one of the following:

• Community-based health professional (Aged Care Assessment Service)
• General Practitioner (GP)

Cost

$80 for Commonwealth Home Support Program (CHSP) clients per program.
