Eligibility

1. Must have one (or more) of the following health problems:

Recent fall
History of falls (in the past 12 months)
Risk of falls
Fear of falls

2. Must live in one of the following areas:

Mornington Peninsula Shire
Frankston Council
Parts of Casey/Cardinia & Kingston Council Areas

3. Must have a referral from one of the following:

Hospital or community-based health professional
General Practitioner (GP)
Self-referral or referral from family members are accepted

Cost

There is no cost to attend the Falls Prevention Service.
