Eligibility

1. Must have one (or more) of the following health problems:

Recent fall
History of falls
Risk of falls
Fear of falls

2. Must live in one of the following areas:

Suburb with the postcode 3888

3. Must have a referral from one of the following:

Hospital or community-based health professional
General Practitioner (GP)
Self-referral or referral from family members are accepted

Cost

The cost to attend the initial assessment is $10.30.

The cost to attend the Falls and Balance Program is $5 per session.
