Services provided

• Falls and Balance Program (1 – 2 sessions per week for 8-weeks)
• Face-to-face

Available Staff

• Physiotherapist
• Occupational Therapist
• Access to Geriatrician and broader allied health team as required

Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls

2. Must live in one of the following areas:

• City of Wyndham

3. Must have a referral from one of the following:

• Hospital or community-based health professional
• General Practitioner (GP)

Cost

There is no cost to attend the Falls and Balance Program.
