Services provided

• Strength and Balance Program (1 session per week for 12-weeks following an initial assessment)
• Face-to-face

Available Staff

• Physiotherapist
• Allied Health Assistant

Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls

2. Must live in one of the following areas:

• No catchment area

3. Must have a referral from one of the following:

• Hospital or community-based health professional
• General Practitioner (GP)
• Self-referral or referral from family members are accepted

Cost

The cost to attend the Strength and Balance Program is $8 per session however individual circumstances are taken into account.
