1. Must have one (or more) of the following health problems:

Recent fall
History of falls
Risk of falls
Fear of falls

2. Must live or work in one of the following areas:

City of Brimbank

3. Must have a referral from one of the following:

Hospital or community-based health professional
General Practitioner (GP)
Self-referral or referral from family members are accepted
Must have a referral from MAC if >65 years old
