1. Must have one (or more) of the following health problems:

Recent fall
History of falls
Risk of falls
Fear of falls
Social isolation

2. Must live in one of the following areas:

City of Boroondara

3. Must have a referral from one of the following:

Hospital or community-based health professional
General Practitioner (GP)
My Aged Care
Self-referral or referral from family members are accepted

Cost

The cost to attend the Strength and Balance Program is $12 per session ($8 with My Aged Care referral).
