Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls

2. Must live in one of the following areas:

• South Gippsland Shire Region

3. Must have a referral from one of the following:

• Hospital or community-based health professional
• General Practitioner (GP)
• Self-referral or referral from family members are accepted

Cost

The cost to attend the Falls and Balance Program is $8 per session or $55 for 8 weeks.
