• Falls and Balance Clinic (1:1 appointment with a follow up review after 3 months)
• Telehealth

• Physiotherapist
• Occupational Therapist
• Rehabilitation Physician

Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls
• Falls where the cause is complex or unclear

2. Must live in one of the following areas:

• Shire of Cardinia
• City of Knox
• City of Maroondah
• Shire of the Yarra Ranges

3. Must have a referral from one of the following:

• Hospital or community-based health professional
• General Practitioner (GP)
• Self-referral or referral from family members are accepted

Cost

The cost to attend the Falls and Balance Clinic is a $10 co-payment per face-to-face visit.
