• Falls Prevention Program (x1 session per week for 7 weeks)

Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls

2. Must live in one of the following areas:

• Melton Shire
• Moorabool Shire

3. Must have a referral from one of the following:

• Hospital or community-based health professional
• General Practitioner (GP)
• Self-referral or referral from family members are accepted
• My Aged Care (if aged over 65)

Cost

The cost to attend the Falls Prevention Program is $5 per session.
