Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls

2. Must live in one of the following areas:

• Colac Otway Shire

3. Must have a referral from one of the following:

• Hospital or community-based health professional
• General Practitioner (GP)
• Self-referral or referral from family members are accepted
• Allied Health Professional

Cost

Balance and Mobility Screening Assessment/Clinic

• $15.70 per session without a Healthcare Card
• $10.20 per session with a Healthcare card (price current as at June 1^st 2022)

Strength and Stability Program

• $8 per group session (price current as at June 1^st 2022)

Falls Prevention Program

• $8 per group session (price current as at June 1^st 2022)

Review of these service costs can occur based on individual circumstances.
