SERVICES PROVIDED

• Seniors Exercise Park High Level Balance group (1 x 1 hour session, weekly)
• Group session
• Face-to-face
• Age criteria: 65 and older

AVAILABLE STAFF

• Physiotherapist
• Exercise Physiologist
• Allied Health Assistant

ELIGIBILITY

1.      Must have one (or more) of the following:

• Age criteria: 65 and older
• Recent Fall
• Past History of Falls
• Risk of Fall
• Fear of Falls
• Be able to walk with or without walking stick for 10 minutes continuously.

2.      Must live in one of the following areas:

• Bentleigh East and nearby area (contact service if unsure)

3.      Must have a referral from one of the following:

• Community-based health professional
• Allied Health Professional
• Aged Care Assessment Service
• Self-referral or referral from family members are accepted
• My Aged Care (if aged 65 and over)

COST

• $10 per session.
