Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls
• Unsteadiness or reduced confidence with balance

2. Must live in one of the following areas:

• No restriction but clients mainly live in the City of Port Phillip

3. Must have a referral from one of the following:

• Hospital or community-based health professional
• General Practitioner (GP)
• Self-referral or referral from family members are accepted

Cost

$10 per session (for most clients) or $30 per session (for Home Care Package clients).
