• Individual therapy sessions are also available

Eligibility

1. Must have one (or more) of the following health problems:

• Risk of falls

2. Must live in one of the following areas:

• Bellarine Peninsula

3. Must have a referral from one of the following:

• Hospital or community-based health professional
• General Practitioner (GP)

Cost

The cost to attend the Make a Move Falls Prevention Program and/or individual therapy sessions is based on individual circumstances.
