Eligibility

1. Must have one (or more) of the following health problems:

Recent fall
History of falls

2. Must live in one of the following areas:

Moira Shire
Berrigan Shire

3. Must have a referral from one of the following:

Hospital or community-based health professional
General Practitioner (GP)

Cost

The cost to attend the Falls and Balance Program is $8 per session.
