Must have one (or more) of the following health problems: Recent fall History of falls Risk of falls Fear of falls 2. Must live in one of the following areas: Ararat East Grampians 3. Must have a referral from one of the following: Hospital or community-based health professional General Practitioner (GP) Self-referral or referral from family members are accepted Cost The cost of the initial assessment is $10.50 or $15.50 (based on individual circumstances).
