Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls

2. Must live in one of the following areas:

• Ararat
• East Grampians

3. Must have a referral from one of the following:

• Hospital or community-based health professional
• General Practitioner (GP)
• Self-referral or referral from family members are accepted

Cost

The cost of the initial assessment is $10.50 or $15.50 (based on individual circumstances).

The cost to attend the Falls and Balance Program is $50.
