Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls
• Feels unsteady on feet, or wanting to prevent future deterioration

2. Must live in one of the following areas:

• City of Knox and surrounding suburbs
• City of Maroondah and surrounding suburbs

3. Must have a referral from one of the following:

• My Aged Care

Cost

The cost to attend the Falls Clinic is $11 per session (can be reduced or waived if needed).

The cost to attend the Falls Prevention Program is $9 per session (can be reduced or waived if needed).
