• Falls and Balance Program (1:1 assessment/treatment as required and 6-week group sessions)
• Face-to-face
• Telehealth

Eligibility

1. must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls

2. must live in one of the following areas:

• Mount Alexander Shire
• Central Goldfields Shire
• Macedon Ranges Shire

3. must be referred by:

• Hospital or community-based health professional
• General Practitioner (GP)
• Self-referral or referral from family members are accepted
