• Falls and Balance Program (Twice weekly, 8 week duration)
• Face-to-face

AVAILABLE STAFF

• Physiotherapist
• Exercise Physiologist
• Allied Health Assistant

ELIGIBILITY

1.      Must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls

2.      Must live in one of the following areas:

• Moonee Valley City and
• City of Melbourne

3.      Must have a referral from one of the following:

• Hospital or community-based health professional
• Allied Health Professional
• Care Co-ordinator or Case Manager
• Aged Care Assessment Service (ACAS)
• General Practitioner (GP)
• Self-referral or referral from family members are accepted

Based on individual circumstances.
