Services provided

• Falls and Balance Program (x1 session per week for 6-week) following an initial assessment
• Individual therapy sessions also available
• Face-to-face
• Telehealth

• Physiotherapist
• Exercise Physiologist
• Allied Health Assistant

Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall
• Past history of falls
• Risk of falls
• Fear of falls

2. Must live in one of the following areas:

• Benalla Rural City

3. Must have a referral from one of the following:

• Hospital or community-based health professional
• General Practitioner (GP)
• Self-referral or referral from family members are accepted

Cost

There is no cost to attend the Falls and Balance Program.

The cost to attend individual therapy sessions is based on individual circumstances.
