Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls

2. Must live in one of the following areas:

• Eastern Metropolitan Suburbs

3. Must have a referral from one of the following:

Integrated Falls Assessment Service

• Hospital or community-based medical professional
• General Practitioner (GP)

 No Falls Exercise Program

• Hospital or community-based health professional
• General Practitioner (GP)
• Self-referral or referral from family members are accepted

Cost

There is no cost for Medicare card holders with a GP referral to attend the Integrated Falls Assessment Service.

The cost to attend the No Falls Exercise Program is based on individual circumstances.
