The directory aims to increase health care professional and community awareness regarding the various outpatient and ambulatory services provided by public hospitals, health services and community health centres in Victoria that are available for community-dwelling older people who have experienced a fall or have been identified as being at risk of falling. It is anticipated that providing a centralised reference point of clinics and programs offering falls assessment, intervention and prevention will improve access to these specialist services Service providers are requested to register the operational details of their outpatient or ambulatory falls & balance clinics and/or programs. This includes information such as the location, eligibility criteria, referral requirements, structure and cost of the service/s.
