The Royal Commission into Aged Care Quality and Safety commissioned NARI to review integrated models of care for older people in Australia. Our report, Models of Integrated Care, Health and Housing considers models at both the individual or clinical level and at the community level. It also considers models of housing for older people with aged care needs. The report discusses how such models could be implemented within Australia’s aged care and healthcare systems, and identifies some policy and systemic barriers to their implementation. Finally, it includes case studies of specific aged care or healthcare providers in Australia and examines what integrated care means in the context of their services.
