Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls
• Feels unsteady on feet or wanting to prevent future deterioration

2. Must live in one of the following areas:

• City of Kingston and surrounding suburbs

3. Must have a referral from one of the following:

• Hospital or community-based health professional
• General Practitioner (GP)
• Self-referral or referral from family members are accepted
• My Aged Care (if aged 65 years and over)

Cost

The cost to attend the Falls and Balance Clinic is $10 per session (can be reduced or waived if required).
